MD Raptors Sport Registration Form

In consideration of my/the minor's participation in any way in any MD Raptor Basketball activities. I, for myself, or on behalf of the minor as parent or guardian, and my/the minor's personal representatives, assigns, heirs, and next of kin: acknowledge and understand that the nature of the athletic activities and the participant's experience and capabilities and believe the participant to be qualified in good health and in proper physical condition to participate in the competitive basketball games as is, without modification, accommodation, or specific personnel. By submitting registration to MD Raptors Youth Basketball, the applicant authorizes MD Raptors Youth Basketball without limitation, the release in perpetuity, without compensation, reservation, or limitation, to allow the reproduction, dissemination, and publication of the applicant's name, nickname, image, voice, signature, facsimile, biological information, photograph, and/or likeness for media coverage, public relations, or any other purpose whatsoever, which may involve the use of pictures, film, videotape recording and/or any other form of media, whether currently in existence or not, and understand that the MD Raptors Youth Basketball retains title, exclusive and unlimited rights to all internet streaming files including live and achieved games, interviews, and events broadcast to the Internet. Maryland Raptors Youth Basketball is under no obligation to exercise the rights herein granted. The applicant understands and agrees that he/she will not receive any payment for the possible commercial use of his/her name or likeness
